Development of database for Community Diagnosis by Community Health Nurse

Community diagnosis is one of the important tasks of community health nurses, and planning for the community health care provides a very good opportunity for them to carry out community diagnosis. To do accurate community diagnosis in limited time they will need a unified Development of a Database for Community Diagnosis by Community Health Nurses common database. Our previous survey found that, in making a Maternal and Pediatric Health Care Plan for the community (in 1996) , every local government carried out community diagnosis.An analysis of the Maternal and Pediatric Health Care Plans eveloped by all the local governments in Okayama revealed that, though community health nurses used data concerning demography, health checks, etc., each local government stored data in diffenrent forms. Moreover, it was not clear how data was processed for community diagnosis. The present study investigated what kinds of data were needed to develop a database for community diagnosis.Community diagnosis is the starting point of, and central to, the Community Health Nursing Process which include planning, implementation, and evaluation. Community diagnosis is also available for the evaluation of community health nursing activities. Each local government layed out the Maternal and Pediatric Health Care Plan in 1996, and also they evaluated the implementation of the plans in 1997. This evaluation will help to improve the diagnosis of each community. However, if each local government continues to use their own forms for their evaluation, it will hinder the standardization of community diagnosis. To develop a standardized (way of) community diagnosis, a unified database was established in collaboration with the prefectural government to assist in making a Maternal and Pediatric Health Care Plan effectively in limited time.
