糖尿病患者の専門治療を行っている数病院において治療,看護場面の参与観察を行い、教育入院及び継続看護に関与している看護婦及び医師、並びに糖尿病による治療を継続中の患者を対象に半構造的面接調査を行った。さらには、糖尿病等の慢性疾患患者へのセルフケア支援を行っている看護婦・士を対象に、アンケート及び自記式調査記録用紙を用いてセルフケア支援をめぐる経験と問題点についた調査を行った。それらの結果について、患者のセルフケアへの内発的動機づけという視点から分析を行い、以下の結果を得た。(1)糖尿病患者のセルフケアが不十分な理由:(1)患者自身に関わる要因(a)病気と療養についての理解不足(病状・生活障害の重さ、セルフケアの必要性)(b)情緒の不安定さ(C)家族関係の不安定さ(2)看護者に関わるもの (a)病気と患者についの理解不足 (b)セルフケア支援とその結果についての認識不足(c)医療チームの連携不足(3)看護者と患者の関係性に関わるもの (a)患者との関係作りの不十分さ(希薄さ、対立、依存関係)(b)認識や目標のずれ(2)糖尿病患者のセルフケア支援をめぐる問題点:(1)患者教育における効果の不十分さと不安定さ (2)患者教育における個別性尊重の不足この結果から、糖尿病患者のセルフケア支援の効果を高める要因として以下の項目が抽出できた。(1)患者の自立を促進する方向での関与(1)患者の自己決定機会の提供(2)患者の有能さの感覚を上昇させる方向での学習支援のシステム化(2)専門性の高い医療チームの形成(1)専門性の高い看護職の配置(2)専門性の高い看護職を核としたチームワーク(3)患者と医療チームとの豊富な対話(a)病状評価・計画立案への患者の参加(b)看護相談の業務としての位置づけの明確化
