デジタルパノラマエックス線画像(CRオルソパントモグラフィ)に描記される頸動脈石灰化が,歯科治療時の循環器リスク評価としての有用性を検討した.対象は,60歳以上の患者31名(男性/女性:13/18)平均年齢76歳(63〜89歳)であった.頸動脈の石灰化は8例(出現率25.8%)にみられた.頸動脈石灰化症例の平均年齢は75±4.8歳,男性3名,女性5名であった.頸動脈石灰化は,第3〜4頚椎近傍にみられた.術前合併症のうち,糖尿病症例に頸動脈の石灰化症例が多くみうけられた(71.4%)次いで,虚血性心疾患(30%)であった.術前の血圧および脈拍数と頸動脈の石灰化とは関連はなかった.抜歯症例について,循環動態を調査した結果,頸動脈石灰化症例では,術中の収縮期血圧が158mmHgで,対照症例(140mmHg)より血圧が上昇した.この上昇は,術前値より23mmHgの上昇であり,対照症例(11mmHg)より,変動が大きかった.脈拍数の変化に差はなかった.本研究の結果より,CRオルソパントモグラフィに描記される頸動脈石灰化症例は、歯科治療中に、血圧の変動が大きいことが予想され、循環器リスク評価として、CRオルソパントモグラフィ上の頸動脈石灰化は有用な指標となりえる.今後さらに,糖尿病患者の循環器リスク評価として検討する必要がある.

To investigate the usefulness of the cycler in dental therapy by imaging the calcified carotid artery with CR imaging. 31 patients (male/female:13/18) aged 60 years or older, with an average age of 76 years (63 - 89 years). Calcification of the carotid artery occurred in 8 cases (occurrence rate 25.8%). The average age of patients with carotid calcification was 75±4.8 years old, 3 males and 5 females. Carotid calcification, near the third and fourth vertebrae. Preoperative complications included diabetes mellitus, carotid artery calcification (71.4%), and heart disease (30%). Preoperative blood pressure, pulse count and calcification of the carotid artery are related. In the case of cervical calcification, the blood pressure during systole increased to 158mmHg, while in the case of contrast (140mmHg), the blood pressure increased. The increase was 23mmHg before surgery, and the increase was 11mmHg after surgery. The pulse number changes and the difference is not obvious. The results of this study indicate that CR is a useful indicator of carotid calcification in patients with carotid calcification, in dental treatment, in changes in blood pressure, and in circulatory evaluation. In the future, it is necessary for patients with diabetes to evaluate their circulatory system.
