Research on structuring the function of home care and clarifying the content of care

本研究的目的是通过对护理需求和家政服务需求的分类和整理，明确家庭护理的内涵和特点。研究分两步进行，第一步，在收集护理及家政服务需求问卷的基础上，对日本12个护理服务站的366名病人进行护理与家政服务的记录。采用多元回归分析方法，对护士就诊、家政服务就诊等影响就诊频度的因素进行分析，包括短诊和长诊两种类型。即使通过其他变量提取影响因素，护理治疗次数和短式就诊次数也会影响护理就诊频率。独居情况、日常生活自理能力较低、家庭成员可供照顾的时间有限是增加家政服务频率的因素。本研究第二步，本研究以某日夜三班制护士出诊服务站98名被试为研究对象。从护理计划和家访记录中收集向使用者提供护理的内容。根据收集的数据，描述了护理需求、所提供的专业和时区。通过护士和家政人员在每个时区提供的护理内容，使服务用途和护士之间的联系迅速发生变化，并提供医疗服务，特别是夜间探视。结论：全天候居家护理通过在家中照顾临终病人，并在客户生病时快速反应，以及在家中提供医疗服务，使更多的患者留在家里。

The purpose of this research is to clarify the meaning and the specialty of home nursing through classifying and arranging the needs of nursing care and home help. The research was done with two steps.As the first step the records of the nursing visits and home help visits for 366 patients of 12 Visiting Nurse Service Stations in Japan based on the questionnaire for collecting the needs for nursing and home help services. By the multiple regression analyses the factors which affect the frequencies of visiting services, such as nurse visits and home help visits of both types of shorter visits and longer visits were investigated. Even though extracting the effects by other variables, the numbers of the nursing treatments and the numbers of visits of shorter type visits affected the frequencies of nursing visits. The factors that increase the frequencies by home helpers were the situation of living alone, lower ADL, and the limitation of the available time for caring by family members.At the second step of this research ninety eight subjects of a visiting nurse service station which provides nurses' visits in day and night by three shifts. Content of provided care to the users were collected from the records of care plan as well as home visits. Based on the data collected, needs for care, the profession and time zone provided were described. Through the content of care provided in each time zone by nurses and home-helpers enabled the quick change of service use and nurses' correspond as well as providing medical treatment especially visiting at night time. As the Conclusion around-the-clock in-home nursing care enabled more clients stay at home through care for dying patients at home and quick response when clients get sick as well as providing medical treatment at home.
