项目摘要

唇顎口蓋裂患者に対する言語治療としての言語管理と構音障害治療および鼻咽腔閉鎖機能賦活法について、効果的な治療技術と評価方法を総合的に体系化することを目的として、今年度では、当院で行っている成人の口蓋裂患者に対する検査法ならびに言語治療技法について検討して以下の知見を得た。(1)鼻咽腔閉鎖機能機能の判定方法:口蓋裂患者延べ350例から得られた鼻咽腔閉鎖機能検査結果をロジスティック回帰分析を用いて解析し、機能獲得の程度を数量的に判定する方法を開発した。(2)機能不良例に対する治療の選択方法:鼻咽腔造影X線写真を用いて安静時と機能時における鼻咽腔周囲の形態を測定することにより、二次手術の適否とその治療予後の予測を可能とする方法を考案した。(3)言語障害治療法a.語音弁別訓練:成人であっても小児と同様に耳の訓練が必要であったが、内容としては正誤弁別のみが単音と会話レベルで必要であると考えられた。b.子音の誘導法と定着法:音の誘導法は基本的には小児に対する方法で十分であり、補助手段としてはダイナミックパラトグラフ、フローネイザリティグラフが有効であった。定着と般化には、患者が構音した音の正否を自身で弁別するよりも、発音器官の操作を内省させる方が有効であった。
