本研究では、ICUに勤務する看護婦の作業負担や交代勤務による生体リズムの変調、さらには勤務経験などがICU看護婦の身体・精神状態に与える影響を検討することを目的とした。某大学病院集中治療部に勤務する平均年齢27.1歳の看護婦15名を対象に調査研究をおこなった。調査内容は自律神経の活動状態の測定を目的とし、GMS社製携帯型心電計AC301を用い、心拍の揺らぎを、尿中カテコラミン、唾液中のコルチゾールなどの測定をおこなった。R-R感覚の周波数分析は諏訪トランス社製Memcaleシステムソフトを用い解析し、0.15〜0.4Hzの高周波成分(HF)を副交換神経活動の指標とし、HFと低周波成分(0.04〜0.15Hz)との比(LF/HF)を交感神経活動の指標として評価した。また唾液はRIA法によるコルチゾールの測定をおこなった。結果、心拍数は、日勤、準夜勤に比べて夜勤では低い値で推移した。HFは睡眠時に高く、LF/HFは覚醒時に高い値を示した。夜勤時のLF/HFは、日勤や準夜勤と比較して差がないか、むしろ高い値を示した。夜勤時のHFは、勤務明けに高くなる傾向がみられ、日勤や準夜勤と比較して高い値で推移したが、生体の概日リズムが影響しているためと考えられた。勤務終了時における自覚症状の訴え率をみると「眠気とだるさ」は日勤35%、準夜勤42%、夜勤58%、「注意集中困難」では日勤19%、準夜勤60%、夜勤65%と夜勤終了時が最も高かった。唾液コルチゾールの変化は、日勤、準夜勤翌日では、朝方に最高値を示す概日リズムがみられたが、夜勤明けの場合、最高値位相のずれが生じていた。夜勤の場合、明け方にかけて交感神経活動の低下と副交感神経活動の上昇、眠気・注意集中困難の訴えが高くなる傾向にあった。この結果から夜勤前の十分な休息時間の確保や勤務中の仮眠を含めた改善策の必要性が示唆された。
