寝たきりの在宅療養者の体位変換頻度の適正化についての実験を行った。高齢女性(平均63.1歳)9名と若年女性(平均19.9歳)10名を被検者とした。実験条件としては拘束状態(2時間安静臥床体動禁止)、無拘束状態(四肢動のみ可能)の2条件とした。実験中は血流量・体圧分布などの測定やビデオによる動作分析をおこなった。結果:血流量は拘束状態では仙骨部、肩胛骨部ともにわずかに増加傾向を示したものもいたが、全体的にみると終了時まではほとんど変動しなかった。無拘束状態では実験開始10分から頭部、四肢の動きによると考えられる仙骨部、肩胛骨部の血液量の増加を認めた。体圧は拘束状態では頭部、肩胛骨部の圧増加が終了時まで認められた。無拘束状態で頭部、肩胛骨部に圧の増減が見られたが仙骨部では終了に至るまで圧の変化はなかった。主観的快適感は、拘束、無拘束状態で時間経過にともない快適感の低下が著しかった。拘束状態では、疼痛、圧迫、痺れなどの身体症状の訴えが30分以降から多くみられた。また老人よりも若年者の訴えが多かった。実験終了時の背部発赤状況は拘束状態で、仙骨部14名、踵部13名、無拘束状態では、仙骨部6名、踵部3名と無拘束状態の発赤は少なかった。ビデオによる動作分析では、10分過ぎから7名の被検者は頭を左右に動かす、四肢の屈伸などの動きがみられ、その他の被検者も20〜30分以降に動きが頻回となった。以上の結果から、寝たきり在宅療養者の看護として、褥創の発生要因となる局所の圧迫や血流量の低下を除去する方法として、頭部を動かす、上肢の挙上、下肢の屈伸、外転などの運動を頻回に行うことで体位変換に準じた効果が期待できるという示唆を得た。
