本研究は、1.脳・心臓疾患を発症した職業を持つ患者の復職に関する体験についての調査・分析、2.復職体験の分析を基盤としたアセスメントツール開発の2段階で行った。1.2年以内に脳・心臓疾患を発症し重篤な後遺症のない通院加療中の患者8名を対象として半構成面接調査を行った。本研究における復職に関する体験とは、職業を持つ患者が発症から労働生活に再適応する過程において、感じたこと、考えたこと、行動したこととして定義し、面接の逐語録を複数の研究者で繰り返し読み、その意味内容から要約した復職に関する体験を類似性に従い分類を行った。その結果、154件の復職に関する体験が示され、【仕事関係者・組織からのサポート】【働くことへの姿勢の再構築】【労働生活再適応を促進するためのセルフマネジメント】【復職に関する自己決定と行動】【健康レベルがもたらす復職の困難さ】【生活基盤確保のための社会資源活用】【労働再開により生じる心身への影響】【健康状態と労働生活のバランスの不確かさ】【家族からの経済的・精神的サポート】の9つのコードに分類された。さらに復職体験の構造化を試みた。2.前項で得られた結果を基にアセスメントツール試案を作成した。入院加療中の患者1名にプレテストを実施し、さらに検討を重ね改良した。開発されたアセスメントツールはA4版3枚からなり、「原職における労働生活」と「復職」に関する2つのパートから構成されている。「原職における労働生活」では発症前までの労働生活を健康との相互関連について理解できることに主眼をおいて項目を設定した。「復職」では復職体験として分類されたコードを基本に項目を設定した。なお、今後、アセスメントツール実用化に向けて検証が必要である。研究対象者には文書で目的とプライバシー保護、協力の有無は自由であることを文書で説明し同意を得た。
