大腸菌発現系であるGST融合蛋白発現システムにより、プロトロンビン(PT)全長およびN末端側のFragment-1に相当するリコンビナント欠損蛋白d594を作成し、抗リン脂質抗体症候群患者由来末梢血よりリンパ球を分離し、リコンビナント蛋白を抗原としてT細胞増殖刺激試験を行った。この結果3名で陽性反応を得ることができたが、リコンビナント蛋白を十分産生させることが困難であり、精製プロトロンビンを用い以下の実験を行うこととした。(1)PTをトロンビンで処理して分解し、Fragment-1(F-1)とPrethrombin-1(Pre-1)を精製した。同様に全長のPT(Whole PT)も精製した。(2)精製したタンパクを抗原とし、固相酵素抗体法により抗PT抗体陽性者15名の血清中抗F-1抗体と抗Pre-1抗体陽性者の有無を検討したところ、抗F-1抗体陽性11名、抗Pre-1抗体陽性者9名、両者陽性5名であった。(3)精製したタンパクを患者末梢血由来リンパ球とともに培養し、Cell proliferation ELISA法によりT細胞の特異的増殖能を測定したところ15名中6名で陽性反応を示した。2名はF-1に、3名はPre-1に、1名はF-1とPre-1の両方に反応を示し、当該部位でのT細胞エピトープの存在が示唆された。(4)これらの患者6名のうち、4名はB細胞エピトープとT細胞エピトープの存在部位が一致しており、2名は一致していなかった。(5)T細胞エピトープの存在部位と患者のHLA-DRB1対立遺伝子ならびに病態には明確な相関は見られなかった。(6)以上の結果は抗リン脂質抗体症候群における抗PT抗体のT細胞エピトープは患者により異なることを示唆しており、今後さらに症例を増やし、より細分化した蛋白を用いて検討をおこなうほか、T細胞クローンの樹立も検討中である。

Escherichia coli development system: GST fusion protein production system, protein protein (PT) full-length and N-terminal Fragment-1 equivalent protein (PT) deficient protein d594 was prepared. Anti-lipid antibody syndrome patients were isolated from peripheral blood, protein antigen and T cell proliferation stimulation test. The results of this study are as follows: 1. (2)Anti-F-1 antibody was positive in 15 cases, anti-Pre-1 antibody was positive in 9 cases, anti-Pre-1 antibody was positive in 5 cases.
