現在までにわれわれは、平成12年度科学研究費補助金により、高血圧患者に発症する勃起不全のメカニズムが、特定の薬剤の副作用によるものか、あるいは原発性病態に関係するものかを、調べる目的で、高血圧自然発症ラット(SHR)を用いて勃起に関する種々の検討を行ってきた。その結果、SHRは降圧剤を投与する前にすでに対照の正常血圧ラット(WKYラット)よりも勃起機能が低下していることを発見した。しかし現在までにわれわれが行ってきた方法は、in vivoの実験ではあるが、麻酔下海綿体神経電気刺激による強制勃起の方法であるため、厳密には自然な状態での生理的勃起を反映しているとは言えない。実際の勃起は、坐骨海綿体筋と球海綿体筋の働きが加わるため、血圧を超えて陰茎海綿体内圧が上昇することが予想される。そこで、ラットを用いて無麻酔覚醒下で陰茎海綿体内圧と血圧が同時に測定できるモデルを確立すれば、生理的勃起に際する陰茎海綿体内圧と血圧の変動が明らかになり、さらに、SHRとWKYラットを比較することにより高血圧患者の生理的勃起機能が解明できると考えた。本年度は、主にSHRを用いて無麻酔覚醒下での生理的勃起モデルの作成と確立に取り組んできたが、SHRでは降圧剤を投与する前にすでに対照のWKYラットよリも勃起機能が低下しているためか、降圧剤投与により血圧が下がると、実際の陰茎海綿体内圧も予想以上に低下する傾向が出ている。
