嚥下障害の診断法としては、一般に嚥下造影検査法が最も精度の高い検査法とされている。しかし、本法は専用の設備内でしか行えず、被曝線量の問題もあるため摂食中の長時間にわたる診断や、回復、悪化に応じて度重なる診断が必要な嚥下障害患者の検査法としては十分利用できる方法とはいえない。一方、嚥下障害のスクリーニング法として頚部聴診法が臨床の場で広く行われている。この頚部聴診法は聴診器を用いて嚥下時に産生される呼吸音や嚥下音を聴取し、特殊な機器を必要とせずに非侵襲的に誤嚥や咽頭部の貯留を判定する方法で、ベッドサイドでも容易に行え、患者が実際に摂食している時にも診断が行える検査法である。しかし、頚部聴診法による検査手技ならびに明確な判定基準は未だ確立されていないのが現状である。そこで本研究では嚥下障害スクリーニング法として頸部聴診法を確立させるために、頚部聴診法の適切な検査手技を決定しならびに本法による診断基準を明らかにすることを目的として進めている。現在までに同意の得られた術前・術後の頭頚部腫瘍患者を対象とし、造影剤嚥下時に産生される嚥下音と嚥下前後の呼吸音を被検者の頚部より採取し、同時に透視装置(TOSHIBA GAW 10A現有)を用いて撮像した嚥下および呼気時の透視画像とともにビデオデッキ(TOSHIBA KXO 80N現有)に記録している。この際、頸部聴診法の手法を検討するため、検出器としては加速度ピックアップ(TEAK 501現有)あるいはをマイクロフォン(Sony ECM-77)を用いて、検査前の喀出の有無、検査食の種類、造影剤嚥下後の呼気の出し方など条件を変えながら記録を行っている。現在、記録された嚥下音、呼吸音を検者である歯科医師ならびに言語治療士に呈示し、嚥下音、呼吸音の聴覚的特徴の判定を進めている。
