项目摘要

痴呆高齢者のせん妄の発症過程を明らかにすることを目的とし、平成14年度は介護老人保健施設の高齢者を対象に入所後数日間の睡眠・覚醒周期への影響をとらえた。これに引き続き、平成15年度は療養型病床群の入院高齢者を対象にデータ収集を行った。対象はN病院の療養型病床群のアセスメント病棟に入院した高齢者6名(男性2名、女性4名)である。平均年齢は81.3歳であり、介護度2と3が各2名、介護度4と5が1名ずつであった。対象は高血圧、糖尿病、骨折、腎不全、リウマチなど複数の疾患を有しており、老人性痴呆の診断を有する者は2名含まれた。せん妄評価尺度(DRS)の点数は0から8点に分布し、入院から3日目までの間にDRS12点以上(せん妄と診断)になる者はいなかった。結果的にせん妄となった高齢者が含まれなかったため、療養型病床群でのせん妄の発症過程を明らかにするまでには至らなかった。しかしながら高齢者のデータ収集と同時に実施した受持ち看護師に対する対象高齢者のせん妄発症の予測とその理由についてのインタビューデータから、療養型病床群の看護師が高齢者のせん妄のアセスメントで注目しているデータが明らかになった。注目データを大別すると、入院時の高齢者の様子、入院前の生活と入院環境とのギャップ、せん妄の既往の3つに分類された。入院時の様子は、見当識障害、意味不明の言動、表情が固い・乏しい、落ち着きがない等から判断していた。生活環境のギャップは、本人の希望しない入院、ADL能力の誤認知覚、入院前の夜間不眠の有無などから入院による高齢者のストレス荷重状況をアセスメントし判断していた。これらは、せん妄を予測し適切な予防ケアを実施する上で重要な情報であり、今後のせん妄アセスメントツール開発の基礎資料として役立つと考える。
