【目的】介護保険制度の施行後、在宅サービスの利用量は増えているが、利用者や家族に対する効果については評価が不足している。本研究は、地域在住の高齢者の確率標本から要介護高齢者をスクリーニングし、その介護者を追跡調査することによって、在宅サービスの利用が介護ストレスの軽減や在宅継続に貢献しているか否かを縦断的に測定することを目的とした。さらに介護保険制度施行前に実施した介護者のパネル調査データと比較し、介護保険前後で在宅サービスの効果がどのように変化しているかを検討した。【対象と方法】2002年に東京都下の一市部において65歳以上の住民の確率標本(10,000人)を対象に日常生活動作と認知能力を調べる調査を実施し、在宅要介護高齢者を把握した。その主介護者に対して訪問面接調査を実施し、1年後に追跡調査を行った(初回調査完了数595、追跡調査完了数526)。このデータベースと介護保険前の1996〜97年に同市で同様な手法により実施したパネル調査のデータを分析に用いた。【結果と考察】1)パネルデータを用いて介護ストレスに対する在宅サービスの効果を検討した結果、在宅サービスの利用が介護ストレスを軽減するような効果は介護保険前だけでなく介護保険後も検出できなかった。サービスの利用量は増えているが、介護ストレスを軽減するには量的にまだ不十分である可能性やサービスの質的な問題、サービス利用に伴うストレスなども考慮する必要がある。2)在宅サービスの利用と在宅継続との関係を検討した結果、在宅サービスの入所抑制効果は介護保険後の方が全体として弱くなっていた。介護保険後のデータでは、身体障害の重度化に伴う入所リスクの増大はホームヘルパーの多用により抑制されることが示されたが、認知障害の重度化に伴う入所リスクの増大を抑制するようなサービスはなく、痴呆介護への対応が不十分であることが示唆された。
