われわれは、薬剤誘発によるラット十二指腸潰瘍モデルで、潰瘍形成過程の早期から細胞周期調節因子p21・p27の発現が増加すること、熱傷ストレスラットモデルで、肉眼的に明瞭な胃粘膜病変形成より早期から胃粘膜虚血が生ずること、この虚血性変化には白血球依存性の微小循環内皮細胞障害が重要であり、接着分子セレクチンが関与することなどを報告してきた。これらの結果を踏まえ、熱傷ストレスモデルで胃粘膜病変の形成過程における細胞周期調節因子の関与を検討することとし、手始めに、熱傷ストレスラットモデルにcaspaseの阻害剤(pan-caspase inhibitor)を投与したところ急性胃粘膜病変の分布が変化したことから、病変の形成にcaspaseが(P21などの細胞周期調節因子を介して)関与していることが示唆された。病変分布が変化した理由の一つに、ラットの胃の部位による厚みの違いが考えられる。すなわち虚血で生じる胃粘膜障害が、caspaseが抑制されることで(病変の発生全てを抑制できなかったものの)粘膜の厚い胃体部で顕在化しにくくなった可能性がある。また、caspase-1は胃酸分泌を抑制するIL-1βの転換酵素のため、caspase-1を阻害すれば胃酸分泌が亢進する可能性があった。しかし、今期の研究では胃酸分泌は変化せず、pan-caspase inhibitorは胃酸分泌の抑制を介さずに急性胃粘膜病変の形成を抑制し、胃体部の粘膜に保護的な作用すると考えられた。同時に行った組織染色で、この胃粘膜病変にapoptosisが関与することも明らかになり、また、熱傷ストレスラットの胃粘膜微小循環を生体内観察法で検討し、酸分泌抑制剤であるHistamine 2 receptor blockerが微小循環障害を改善することも示せた。この手技と結果も今後の細胞周期調節因子の検討に応用する。
