项目摘要

本研究では関節円板前方転位と転位無しが混在する片側性関節円板前方転位症例に着目し、転位無し関節の予後から切歯路角、顆路角の関係が円板転位に及ぼす影響を明らかにすることを目的とした。過去(平成8年〜)に収集した顎運動データ、MRIによる顎関節部の形態的特徴データをデータベース化し、その一部をGNYAP (The Greater New York Academy of Prosthodontics 2003)、平成15年度みちのく歯学会,ICP (International college of prosthodontics 2004)にて報告した。また,日本学関節学会雑誌等にその成果の一部を報告した.概略を以下に記す。<初年度(初診時)データ分析>下顎運動測定:片側性関節円板前方転位症例、両側転位無し症例の切歯路角、顆路角を求め、片側性転位症例の顆路角は切歯路角よりも大きいことが確認できた。<予後データ分析>調査対象が初診時の状態でMRI検査結果から片側性関節円板前方転位と認められた34名中,本実験の趣旨,および予後データの収集に同意の得られた10名について再度MRI撮像を行った.予後データ(MRI撮像)収集までの平均経過年数は2年2ヶ月であった.本実験において,関節円板の転位状態に変化が認められた被験者は認められなかった.すなわち,本実験結果から,切歯路角,顆路角の関係から円板転位のリスクファクターを解明するには至らなかった.このような結果に至った考察として,予後データの測定までの期間が平均2年2ヶ月と短期間であったこと,平均年齢が42.3歳であったことがあげられ,さらに長期的なスパンで経過を追う必要があること.顎関節症の後発年齢である20歳代をターゲットに被験者を収集する必要性が感じられた.今後は,本実験の被験者を継続して経過観察すると共に,さらに20歳代の被験者をターゲットに追実験を行う予定である.
