本研究の目的は、安全な医療提供を促進するための看護職を含む医療専門職およびその学生に対する教育訓練プログラムを開発することである。今年度は、具体的な教育訓練プログラムを開発すると共に、プログラムで使用する教材の開発を行った。前年度の研究結果から、安全な医療提供を促進するために医療専門職者が、職場・作業環境や人間関係、システム上の不虞について認知できるような教育訓練プログラムの必要性が示唆されていた。また、通常業務の中に潜むリスク要因を事故発生に至る前に捉えられる能力を高めることができるような、実践状況をイメージできる教育ツール活用の重要性も指摘されていた。そこで今回は、教育訓練プログラムの対象者が事例の分析を通じて職場・作業環境や人間関係、システム上の不虞を継続的かつ反復して分析できるように、これらの要因を含むシミュレーション事例を活用した教育訓練プログラムを作成した。作成の過程では、実践家の協力を得て、通常業務の中で頻繁に出会うインシデント事例を集め、インシデントの直接原因となる医療行為者の問題のみならず、職場・作業環境や人間関係、システム上の不虞を含む教育訓練用のシミュレーション事例を計6事例作成した。作成した事例は、教育訓練の対象者が状況をイメージしやすくなるように文字だけではなく、状況を示したイラストを用いて説明するようにし、これらの事例に分析の際に検討する共通の質問を加えて教育訓練プログラム用テキストとした。作成したプログラムは、実践家にそのわかりやすさ、使いやすさの評価を依頼した。教材の使用に対しては、概ね良好な評価を得たが、教育訓練の際の指導要領の作成や教育訓練を実施するトレーナーの養成においては、課題が残った。今後は、プログラムを精錬すると共に、トレーナーの養成やプログラムを客観的に評価するツールの開発を進めていくことが必要である。
