1.妊娠期のセルフケア行動の測定と健康評価、看護介入(1)助産院における妊娠期のケア都内、近郊、北陸地方の助産院4ヵ所において妊娠期のケアについて調査食事、運動、身体の観察の仕方、冷えの予防と温熱療法、安産に向けた健康管理、などの項目が抽出された。(2)セルフケア行動に関する評価尺度は信頼性・妥当性の検討が十分とはいえず、健康評価尺度の有効性が見出された。(3)看護介入においてはその評価としてのエビデンスが十分とはいえないが、(1)におけるケアの実施により、安全でかつ産婦と家庭の満足度の高い出産となることが経験上明らかである。しかし、看護介入の評価としてのエビデンスは十分とはいえず、効果の検証が必要である。2.出産施設の違いによる妊娠期のケアと分娩結果に関する国内外の文献検討(1)海外の結果The Cochrane Libraryによるシステマティックレビュー2編と進行中のプロトコール1編の検討を行なった。家庭的施設分娩vs.一般的施設分娩のシステマティックレビューにおいては、いくつかの項目において家庭的施設分娩にわずかな利点が認められた。ケアの評価は、出産場所の評価から、施設内機能別システム評価、ケア提供者の職種の違いを含むケア提供内容へと変化している。(2)国内の結果助産院と病院の比較を検討したところ、国内における現状ではエビデンスとなる客観的データが十分とはいえない。今後のアウトカム指標として、安全性と快適性の両面からの指標を用いた研究が望まれる。(3)妊娠期の看護介入妊娠期のセルフケア行動は、助産師のケア内容に大きく影響を受けており、看護介入の評価としてのアウトカム指標が見出された。
