本課題の最終目的は、短時間一回完結形式の認知行動療法（ACT：Acceptance and Commitment Therapy）によって、心理療法的介入を糖尿病臨床に導入する際の高コスト問題を解決することである。今年度の研究では、前年度の研究知見を踏まえて、以下の2つの研究を実施した。（e）2型糖尿病患者を対象に、短時間一回完結形式の認知行動療法を実施した。その結果、前年度に作成した質問紙（日本語版 Acceptance and Action Diabetes Questionnaire）の得点の推移から、ACTが重視しているアクセプタンス方略が向上していることが示された。糖尿病関連指標の推移から、少なくとも介入後2ヶ月の時点において、血糖コントロールが有意に改善しており、小から中程度の効果量が示された（研究5：国際学術雑誌に投稿予定）。（f）Ecological Momentary Assessment（EMA）による日常生活下調査の結果をフィードバックすることで、個別の患者ごとに血糖コントロールに関わる要因を同定し、それらの要因に対して介入を実施するための方法論を示した（研究6：国際学術雑誌に投稿予定）。本課題の成果は、2型糖尿病患者に対する効果的な介入プログラムを作成するための知見を提供しており、心理療法的介入を糖尿病治療の現状に即した形で導入することが期待できるという点において、意義深いと考えられる。最後に本課題の限界と今後の展望について述べる。研究5、研究6の2型糖尿病患者は、平均的な糖尿病患者と比べて、血糖コントロールは同程度であったが、やや若年の患者が多く、合併症の発症者は含まれていなかったことがある。今後は、高齢の患者や合併症を発症している患者を含めて、検討を続けていく必要がある。
