项目摘要

本研究は、日本における慢性病者に特有なセルフケア能力の構成概念を明らかにすることを目的として実施した。平成15年度に続き、平成16年度は、看護師と患者の双方の視点を取り入れた「日本における慢性病者に特有なセルフケア能力の構成概念」の原案を精錬させ、新しいセルフケア能力の質問紙(新Self-care Agency Questionnaire;以下、新SCAQとする)を作成した。新SCAQの構成概念は、【健康管理への関心】【健康管理方法の選択】【体調の調整 -弱みと強みを考慮して-】【健康管理法の継続】【支援の活用】、そして【前提となる能力】となった。新SCAQは、質問項目の他に構成概念毎に自由記載欄を設けて、対象となる人の個別性を記載できるようにした。日本における慢性病者に特有なセルフケア能力の構成概念、すなわち、新SCAQの構成概念の妥当性を検討する調査を実施した。慢性病者のケアに精通している看護研究者5名を対象として質問紙調査を実施し、新SCAQの構成概念や質問項目についての意見を尋ねた。また、慢性病者3名を対象として、新SCAQの回答後に面接調査を実施した。さらに、看護師10名を対象として、受け持ち患者のセルフケア能力を新SCAQで測定した後に、面接調査を実施した。その結果、構成概念の内容妥当性と表面妥当性について、ある程度の支持が得られた。患者は、新SCAQを用いての個々に合わせたケアの提供を期待していた。看護師は、新SCAQの構成概念は慢性病者のケアの指針になると指摘した。【前提となる能力】は他の構成概念とは特色が異なるため、構成概念には含めないこととした。今度の課題は、(1)【健康管理方法の選択】は患者への情報提供が少なく医療者が選択する傾向もあること、(2)【健康管理方法の選択】と【健康管理法の継続】の識別、(3)【体調の調整 -弱みと強みを考慮して-】の「弱みと強み」という表現の洗練、を検討することである。

This study aims to clarify the concept of the composition of the ability unique to chronic disease in Japan.
