脳血管障害の病者や認知症高齢者を内包する家族がどのような知恵や技術を発展させながら生活の再構築に取り組んでいるのか、又家族の知恵や技術に関して実態を明らかにし、看護者としての援助方法を提案することを目的とした。脳血管障害の病者や認知症高齢者を主に介護している者を対象に、データ収集し、継続比較法のもとで分析を行った。分析の結果、生活の再構築に取り組む家族の介護キャリアが導かれた。生活の再構築に取り組む家族の介護キャリアの構造を説明する理論的モデルとして【納得できるストーリーづくり】、【闘う姿勢の確立】、【知を動員した介護の展開】、【調和への希求】、【自己信頼の場の確立】の5つの局面を有するモデルが明らかになった。すなわち、生活の再構築に取り組む家族の介護キャリアとは、慢性状態の軌跡の中で、【納得できるストーリーづくり】をし、【闘う姿勢を確立】して、【知を動員した介護を展開】し、【調和を希求】し、【自己信頼の場の確立】をしていくことである。【納得できるストーリーづくり】として、《介護の意味づけ》、《介護へのコミットメント》、《ストーリーとしての介護の表現》、【闘う姿勢の確立】として、<攻めの構え>、<負けないという念力の保持>、<忍耐>、<介護への意地>、<強固な姿勢での介護への没頭>、<必死の介護>、【知を動員した介護の展開】として、《考えながらする知恵の発展》、《場に応じたツール・法則を見極めたパターンの形成》、《オープンなつながりの形成》、《介護への布石》、【調和への希求】として、《その人らしさを大事にしたかかわり》、《自分らしさの保持》、《自分の中のマネジメント》、【自己信頼の場の確立】として、《コントロール権・主導権の獲得》、《揺るぎない存在の確立》、《家の流れの創造》、《ベテランとしての自信の獲得》のカテゴリーが見出された。また、生活の再構築に取り組む家族の介護キャリアを形成することができない家族では、【悪循環】の局面を辿ることが明らかになった。【悪循環】として、《介護困難という意味づけ》、《オープンになることへの抵抗》、《調和のゆらぎ》、《自信の剥奪》、《自己信頼の場の崩壊》のカテゴリーが見出された。看護への示唆として、悪循環を断ち切ることへの支援という援助の視点が見出され、家族の日常生活・セルフケアの強化、情緒的支援の提供、家族教育、対処行動や対処能力の強化、家族関係の調整・強化、コミュニケーションの活性化、役割調整、親族や地域社会資源の活用、危機への働きかけ、意思決定の支援・アドボカシーといった家族看護エンパワーメントモデルを活用して、介入していくことを提案した。
