在宅要介護高齢者の自然排便を効果的に促進するための訪問看護技術を明らかにすることを目的として、在宅要介護者に対する排便援助で効果のあった事例を経験している訪問看護師を対象として援助過程について聞き取り調査を実施した。その結果、以下のことが明らかになった。訪問看護師が行っていた排便援助は、(1)要介護者の、排便方法の希望、排便及び排便を介護・援助されることに関する気持ち・考え、排便・排便動作の障害、身体精神機能等、介護者の、排便及び排便を介護する・援助されることに関する気持ち・考え、介護状況等を把握する、(2)要介護者の排便方法の希望を実現できる方法を見出して提案する、(3)早期に練習を開始して根気よく継続する、(4)薬剤の使用をなるべく減らして食・飲水・運動・腹部マッサージ等により排便を促進する、(5)失敗を肯定的に受けとめることができるように支える、(6)要介護者・介護者の状態に合わせて排便管理がだんだん自分たちでできるようにしていくという援助を行っていた。効果としては、要介護者の排便方法の希望、従来の排便習慣に近い排便が実現しており、排便障害の改善、排便動作の自立、生活の改善・拡大、家族介護者の排便介護負担の軽減、排便介護方法の習得、生活の改善・拡大等という改善がみられており、自然排便を促進することは時間・根気を要することではあるが、その意義は要介護者・家族にとって大きいと考えられた。
