1.結果1)福島県須賀川市対象者は4ヶ月間に母子健康手帳を交付された222人で、ベースライン調査回答率は93%(206人)、産後約6週目の追跡率は68%(140人)。ベースライン調査での計画外妊娠の割合は20%(42人)。計画外妊娠は以下5項目の危険性を有意に高めていた:母児愛着度が低い、育児に自信があると言えない、子どもを虐待していると思うことを否定できない、子どもとゆったりと過ごす時間があると言えない、子どもの父親が育児に協力的と思わない。カナダで普及しているNobody's Perfectプログラムを参考に、育児に前向きに取り組めることを目的として、母親の自主性を重んじる約2時間の話し合い中心の育児教室を月1回の頻度で計5回、母親20人を2期に分けて実施した。2)ベトナム国ホーチミン市対象者は初回妊婦健診を受診した155人で、ベースライン調査回答率85%(132人)、1ヶ月健診時の追跡率は65%(86人)。ベースライン調査での計画外妊娠の割合は10%(13人)。計画外妊娠は母児愛着度・子育てに対する母親の態度・父親の協力に関連していなかった。頻度の高い「育児の自信があると言えない」に関連する要因は、初産と高学歴であった。3)2国間比較須賀川市はホーチミン市に比較して、母児愛着度および子供とゆったりとすごす時間がある割合が有意に低い一方で、育児に自信がある割合は有意に高かった。ホーチミン市では須賀川市に比較して中絶経験者割合が有意に高かった。2.今後の方向性1)須賀川市:計画外妊娠を含む育児困難を系統的に早期スクリーニングする方法を考案し、試験的に実施している育児教室を評価・検討する。2)ホーチミン市:計画外妊娠の多くは中絶され、出産に至るケースは比較的育児環境が整っていると考えられた。育児困難の関連要因をさらに検討するとともに、須賀川市同様の育児教室の導入を試みる。
