项目摘要

本研究は,外来通院している高齢在宅酸素療法患者を対象に,療養生活における自己管理能力に影響を及ぼす因子を明らかにし,その査定に基づいた患者の行動変容を促すような外来教育プロトコールを検討することを目的に取り組んでいる。本研究では,自己管理能力を,認識(自己効力感)と実行力で捉えることとしている。これまでの調査の結果から,認識(自己効力感)や実行力には,生活充実感や日常生活の自立度,支援体制,病態の重症度,自覚する息切れの程度などが影響していた。また,患者から聞き取り調査を行った結果,呼吸法を修得している患者は,多くの酸素消費を要する場面や自分の体調に合わせて呼吸法の調整を行っていた。つまり,自らでその場面を選択し,意識的に呼吸調整を行っている傾向が見られた。これらから,呼吸法の修得状況が療養生活を左右する因子ではないかとのことが推察された。これらの結果を踏まえて,自己管理能力を査定するための教育プロトコールを作成した。このプロトコールでは,自己管理能力と呼吸法の修得状況などから患者を8段階に分類し,そのレベルに応じた教育プログラムを実施する事とする。教育プログラムは,これまでの調査で得られた自己管理能力に影響を及ぼす因子を踏まえ,各レベルの不足する因子を強化するためのプログラムとなる。今後は,得られた結果をもとに,実際に教育プログラム(パンフレット・ビデオ)を作成し,外来教育プロトコールにのっとった患者教育を実施し,その効果を検証していく予定である。
