项目摘要

18年度報告近年,注意障害に対する認知リハにおいてパーソナルコンピュータ(PC)を利用する報告が散見される(Woodら1987,Ponsfordら1988)が,その妥当性及び効果について充分な検討を行った報告は意外に乏しいのが現状である.このような中で筆者らは,注意障害に市販のPC用ソフトを利用した認知リハを実施した結果,比較的良好な結果を得たことを報告した(窪田ら2006).しかし市販のソフトは,訓練の難易度設定が画一的になる傾向にあり,学習効果が生じ易くなる等の問題があった.そこで今回は,それらの諸問題を改善する目的で筆者は新たに独自にPC用プログラムComputer-assisted Attention Training(CAT)を作成し,それを利用した認知リハをABA'designで実施した結果,ある程度の注意と日常生活動作の改善が認められた.今回筆者が独自に作成したCATの特徴をまとめると次のようになる.1.PC操作自体は知的機能検査の境界域症例でも実施可能であった.2.対象者の重症度は,注意障害が重度であっても担当者が同席すれば実施可能であり,また中等度〜軽度であれば自主訓練が可能であった.3.各課題において難易度設定や患者の反応の記録が容易で,段階付けされた訓練と訓練直後のフィードバックが可能であった.これは訓練への動機づけと訓練時間の短縮にもつながったと思われる.4.欠点として,PCを設置する専用の部屋の確保及びPCの台数の確保が必要と思われた.以上,今回注意障害患者に対する独自に作成したPC用プログラムCATは,PC利用の認知リハとしてある程度の有効性が示唆された.今後さらにこの方面の知見を蓄積し,効果的な認知リハの開発に寄与したいと考える.

18 Annual Report In recent years, attention has been paid to the lack of awareness of the problem and the use of reports (Wood, 1987,Ponsford, 1988) on the adequacy and effectiveness of the problem. The author noted that the market for PC software was damaged and that the results were relatively good (Kuta 2006). The difficulty of training is set to the tendency of drawing a picture, and the learning effect is easy to produce. This paper aims to improve the quality of PC application and Computer-assisted Attention Training(CAT). This paper presents the characteristics of CAT independently. 1. PC operates the self-aware function to investigate the possible implementation of the case. 2. The severity of the subject, the attention to the severity of the impairment. 3. The difficulty of setting the difficulty of each subject and the ease of recording the patient's response. Stage pay is not training, training is not training. The training motivation and training time are shortened. 4. The PC settings are necessary to ensure the number of PCs. The above is an indication of the extent to which the PC is used by patients with disabilities. In the future, the knowledge of this aspect will accumulate, and the knowledge of the results will be developed.
