项目摘要

1.国内外文献レビューアドバンス・ディレクティブの啓発活動の効果を明らかにするため、文献レビューにより介入プログラムを検討した。MEDLINE、CINAHLで無作為介入研究を検索した結果、対象文献は21件でこのうち高齢者を対象とした研究は11件と過半数を占め、介入方法はパンフレットの使用、家庭訪問、コンピューターを用いた介入、カウンセラーによる教育などで、アウトカム指標としてはアドバンス・ディレクティブの作成率、話合いの有無、啓発活動の受け入れ及び満足度等であった。2.介入プログラムの作成終末期ケアの啓発活動に関する高齢者のニードの把握及び効果的な教育方法を検討する目的で、グループ・インタビュー法による予備調査を行った。その結果、延命治療およびリビング・ウイルの是非に関する意向が明らかとなった。さらにアドバンス・ディレクティブの支持及び啓発活動への関心に関連する要因と文献レビューの結果に基づき、ビデオ鑑賞・講義・ディスカッションからなる介入プログラムを作成した。コントロール群には同様のパンフレットの配布のみを行った。3.高齢者に対するアドバンス・ディレクティブの啓発活動の効果健常な高齢者(N=121;介入群60、コントロール群61)を対象に、アドバンス・ディレクティブの啓発活動に関する介入プログラムを実施し、介入前後、1ヵ月後に評価を行った。その結果、介入群では延命治療の意向について「医師の判断」から「希望しない」、アドバンス・ディレクティブの支持が高まるなどの変化がみられた。さらに、家族や医療従事者との終末期ケアに関する話合いについて約2倍の増加を認めた。また、アドバンス・ディレクティブの知識については両群で効果が認められたが、対照群では1ヶ月後に低下傾向を認めた。介入プログラムの全体評価については、10段階中、介入群8.55とコントロール群7.50に比較して有意に高い結果であった。
