健常者ならびに有病者(摂食障害者含む)に対する咀嚼運動後の脳波測定値と心理テスト結果との相関関係の検討我々は,健常成人20名に対し,精神身体的状態を2種類の心理テスト(State-Trait Anxiety Inventory,以下STAIおよびProfile of Mood States,以下POMS)により評価した。さらに圧痛計を用いて,茎状突起部に対する圧痛閾値測定を行い,精神身体的状態が顎顔面部における圧痛閾値測定結果に及ぼす影響について調査した。各被験者から得られた心理テストの各因子の得点と圧痛閾値の変動係数との相関を検討したところ,茎状突起部における圧痛閾値の変動係数とSTAI,POMSの否定的感情に関連した5つ因子の得点との間に有意な正の相関が見られた。本研究結果から,口腔顔面痛を有する患者は精神身体的状態により,筋の触診などの検査結果に影響を生じ,誤った検査結果を導き出してしまう可能性があることが示唆された。また,これらの予備的研究を踏まえ,咀嚼運動後の脳波測定および心理テストを実施し,有病者では,摂食時の咀嚼運動後の脳波測定値と心理テスト結果との間に負の相関がみられ,健常者に比べ有病者では,摂食行動時の脳活動が低くなる一方,精神状態の変化が大きくなることが,示唆された。本研究で使用した心理テストは日常の歯科臨床においても比較的導入しやすく,患者の精神身体的状態を簡単に把握できる反面,歯科医療側の努力で本来解決できる問題を安易に患者の精神的な問題であるかの様にすり替えてしまうとの指摘もあり,歯科臨床における心理テストの実践的な活用においては,今後さらなる検討が必要である。

A study of the correlation between the results of psychological tests and the values of post-mastication exercise in healthy adults and patients (including those with eating disorders) was conducted. In contrast, 20 healthy adults were evaluated for two types of psychological tests (State-Trait Anxiety Inventory, STAI and Profile of Mood States, POMS). In addition, the measurement of pressure threshold value for stem protrusion was carried out, and the measurement results of pressure threshold value for mental body and jaw were investigated. The results of this study showed that patients with oral facial pain had a significant impact on their mental and physical state, and that the possibility of erroneous examination results was also indicated. This study uses the introduction and comparison of psychological factors in daily dental clinic to simplify and grasp the mental and physical state of patients, and the efforts of dental medical side to solve the problem of mental problems of patients are necessary in the future.
