本研究は、手術看護領域の看護師が直面する倫理的課題とその対応を明らかにし、倫理的実践モデルを作成することを目的としている。18年度は2施設より研究協力が得られた。(他1施設依頼中)まず、看護管理者である手術室師長に、(1)手術室のマネージメントの中で倫理的課題(おかしいと思ったり、どうしていいのかわからなくて悩んだ状況)に直面した(または報告された)経験、(2)他部門の看護師や他職種(外科医・麻酔科医)および他の管理職と、どのように連携を取っているか、(3)患者様やその家族へは、どのような対応を取っているか。について面接調査を行なった。面接で得られたデータは個人が特定できないようにコード化し、匿名性を保って逐語録化した。次に、スタッフナースに、(1)臨床実践で直面した倫理的課題の経験、(2)そのときの対応、(3)看護管理者に望むサポート、について面接調査を行なった。面接で得られたデータは個人が特定できないようにコード化し、匿名性を保って逐語録化した。看護管理者が感じる倫理的課題とスタッフナースが感じる倫理的課題の内容には相違が見られた。スタッフナースは倫理的課題に直面した際、看護管理者のサポートが必要と感じており、看護実践の質の向上のためには、看護管理者の倫理的課題への介入が必要であることが明らかになった。本研究の結果の一部は、平成18年9月に行なわれた第16回日本保健科学学会学術集会で発表した。
