项目摘要

昨年度、調査した結果をもとに、電子カルテを導入した中小規模病院における問題・課題を明らかにするために中小規模病院(30床〜292床)で働く看護師433名を対象に、アンケート調査を行った。内容は主に電子カルテ導入前後における業務の変化、電子カルテ使用状況・環境、不便に感じる点・および個人情報保護への配慮についてである。看護業務全般について、「良くなった」と感じている者は7割以上であり、主な看護業務12項目別にすると、『職種間の情報伝達の迅速性』『記録の効率化』『職種間の情報伝達の正確化』『看護計画の立案』については6割以上が「良くなった」と感じており、他いずれの項目も「悪くなった」と回答した者より多い結果となった。しかし、『患者とのコミユニケーション』『残業時間』については、5割以上が「変化なし」と回答しており、新システムの導入によりベッドサイドまでPCを携えての記録は、その操作に慣れるまでは意識がPCに傾き、患者だけに集中できない思いがあるとわかった。さらに、不便に感じる点については、多くはパソコンに関する不便さを感じており、その内容は、5割以上が『起動が遅い』『動き遅い』『フリーズする』と回答しており、4割以上が『パソコン台数の不足』、3割以上が『バッテリー消費の早さ』『パソコンの故障』、またシステムに関して『一患者の同時入力できない』などを感じていた。そういった状況が記録を後回しにさせるなど残業時間の減少とならない理由と考えられた。また、個人情報の保護に関する意識では、個人情報保護法を8割近くは知っていたが、パソコンの画面について、離席時に離席画面にしたり、カバーを閉じたりといった対処をしている者はわずか15%に留まり、「しない」「忘れることがある」とそれぞれ4割以上の者が回答していた。その理由は、4割以上が『患者優先してしまう』、3割以上が『うっかりわすれる』であり、実際に個人情報が守れていないことについて危惧するスタッフも多かった。以上の結果から、電子カルテを導入したことで、看護業務面は多く改善されてはいるものの、ハード(パソコン周辺機器)面、個人情報保護面で、システムそのものや、運用面で様々に改善すべき点が見出された。
