看護管理の視点から臨床現場におけるリスクマネジメントや安全対策、勤務体制の検討を行う際に、看護職者の疲労を客観的に把握することは極めて重要である。看護職者の身体的疲労をより適切に評価するため、平成17年度は運動負荷による疲労モデルを用い、新たな疲労の指標として尿中に排泄される各種サイトカインの種類と変動を検討した。平成17年度の研究成果をもとに平成18年度は、臨床に勤務する看護職者を対象に、尿中サイトカインの測定と質問紙を用いた主観的な疲労指標との関係を明らかにすることを目的に調査を実施した。本研究の対象者は、一般病院に勤務する健康な女性看護師118名であった。看護師の仕事ストレスは看護ストレス尺度(NSS)により評価され、主観的な心理状態は、気分状態プロフィール短縮版日本語版(POMS-SFJ)により評価された。対象者は、各NSS下位尺度のストレスレベルにより高低ストレスグループに分類され、対象者の尿中angiogenin (ANG)、interleukin (IL)-8レベルと主観的な心理状態はグループ間で各々比較された。NSS下位尺度の「患者の生命ケアのための責任の重圧」(PPLC)に関する全被験者の得点は、NSS下位尺度の中で最も高かった。NSS下位尺度のPPLCと「医師との関係困難」において、高ストレスグループは、低ストレスグループより有意に高い尿中ANGレベル(p<.05)とPOMS-SFJの「疲労」得点(p<.01)を示した。尿中IL-8レベルにおいてグループ間の有意差はなかった。本研究の結果は、一般病院に勤務する女性看護師のあいだで高い仕事ストレス、特にPPLCや「医師との関係困難」の観点から、冠動脈心疾患のような炎症性疾患と関連するANGの発現を引き起こすかもしれないことを示唆する。尿中ANGを用いたストレス評価は、看護師の健康問題の予防に役立つかもしれない。

The viewpoint of nursing management is very important from clinical field to safety policy, service system review, nursing staff fatigue and customer control. For an appropriate evaluation of the physical fatigue of caregivers, we will use fatigue indicators due to exercise load in 2017, new fatigue indicators, and the types and movements of various services excreted in the urine. The results of the study in 2017 and 2018 are as follows: There were 118 healthy female nurses in this study. The nurse's career status is based on the NSS, and the nurse's mental status is based on the POMS-SFJ The subjects were classified according to their urinary angiogenin (ANG) and interleukin (IL)-8 levels, and their psychological status was compared according to their respective NSS levels. NSS lower scale of "patient's life and responsibility"(PPLC) related to the total score, NSS lower scale of the highest NSS lower scale PPLC and "physician and relationship difficulties" in the middle, high, low, intentionally high urine ANG in the middle (p<.05) and POMS-SFJ "fatigue" points (p<.01). IL-8 in the urine is a very important factor in the development of the disease. The results of this study suggest that there are significant differences in the status of female nurses in general hospital care, and in PPLC in particular, and that there is an association between coronary heart disease and inflammatory disease and the occurrence of ANG. The use of ANG in urine is a critical factor in preventing health problems for caregivers.
