平成20年度は、入院医療の場、つまりは精神科病棟を含む一般病棟において、看護師が遭遇した患者からの暴力行為についての質問紙調査を行った。結果として、(1)患者の特徴、(2)看護師側の背景の特徴が見出された。(1)の患者の特徴としては、一般病棟での暴力行為は、患者がせん妄状態等や認知症、意識レベルの低下など認知機能が低下した状態で多く起こっていた。また、暴言等の精神的暴力に関しては、先述の患者の特徴に加え、医療側への不信が強い、治療や予後に関して不安を抱えているという特徴があった。(2)の看護師側の背景では、精神科病棟では一般的になりつつある「暴力」の話題ではあるが、精神科病棟以外のいわゆる一般病棟(身体科)では関心が低く、回答した看護師の多くが「暴力」の場面に直面しているものの、患者個々の問題、もしくは看護師自身の資質の問題として捉えていた。そのため、暴力の場面について他のスタッフと共有するものの、体系化されていなかった。このことは、暴力の予防にも関連している。先行研究では、精神科急性期病棟での患者からの暴力について、看護師は様々な情報から暴力の判断を行っていた。対してヒアリングで得られた結果からは、看護師は患者の状態(認知機能の低下、性格など)を把握し「暴力の危険性がある」としながらも「暴力を未然に防ぐ、最小化する」ための方策がとられていなかった。このことからも、「起こった暴力」から、暴力について体系化するとともに、「暴力の予防、最小化」にむけた看護師の判断、方策の充実が不可欠である。
