本年度は、平成18年4月の介護保険制度改正に伴い、要支援1〜2を対象に導入された介護予防サービス(予防給付)の影響として、介護予防訪問看護の利用者の実態を把握することを本研究の目的とした。正式に契約している某市より平成18年4月以降の全介護保険レセプトデータの提供を受け、データベース理論をもとに構造分析を行い、疫学研究が可能なデータベースに加工するとともに、平成18年3月以前のデータベースとの突合を行なった。本データベースを用い、平成18年4月から平成19年3月までの介護予防給付利用者数の推移、および介護予防訪問看護利用者について、介護予防訪問看護利用前月の利用サービスおよび介護度、他のサービスの組み合わせ状況、6ケ月後の介護度の変化等を検討した。平成18年4月の全介護保険サービス利用者数は1,205人であった。うち訪問系サービス利用者(うち要支援1〜2利用者、介護予防利用者)は、訪問介護が255人(52人、8人)、訪問入浴介護が44人(0人、0人)、訪問看護が90人(7人、1人)、訪問リハビリテーションが8人(1人、0人)、居宅療養管理指導が39人(0人、0人)であった。介護予防訪問看護利用者は、平成18年4月が1人、5月が3人、6月が5人、7月が4人、8月が7人、9月が8人であり、10月以降は8人〜10人で推移していた。平成18年4月から平成19年6月までの新規介護予防訪問看護利用者10人の新規利用月前後の介護度の変化をみた結果、介護予防訪問看護導入時に10人中7人の介護度は軽度のレベルに移行していた。介護予防訪問看護導入後6ケ月後の介護度の変化は、10人中7人は介護度に変化はなかったが、3人は介護度が悪化(要支援2→要介護3、要支援2→要介護4、要支援2→要介護5)していた。介護度悪化については、病状の変化による影響の可能性があり、さらなる分析が必要である。

Analysis of the possibility of the impact of the change in the degree of care is necessary.
