本年度は、要介護認定において要支援1・2に該当する、いわゆる虚弱高齢者に対する尿失禁の予防・改善を含めた運動器の機能向上のための有効なプログラム方法を開発することを目的とした。A県B市内3ケ所の通所サービスを利用している要支援1・2に該当する高齢女性に対し、骨盤底筋体操、筋力向上トレーニング、健康教育等を組み合わせたプログラムを週1回の集団指導および自宅でのホームプログラムの実施を3ケ月間提供したところ、以下の知見を得た。1.対象者29名(平均年齢82歳)のうち、尿失禁がある者は16名(55.2%)であった。2.これまで、一般高齢者における尿失禁予防・改善のための介入効果は、約6割という報告があるが、本研究対象者では約5割が運動プログラムに参加後、「尿失禁がなくなった」、または「改善した」と回答し、同等の効果が得られた。よって、予防給付の対象となる虚弱な高齢者に対しても、運動指導介入を行なうことによって尿失禁の改善が図れる可能性が示唆された。3.尿失禁の改善、排尿に関する不具合以外の改善として、「身体が動きやすくなった」「つまずきにくくなった」など、特に下肢の運動機能の改善について自覚している高齢者が多かった。4.予防給付における運動器の機能向上プログラムの提供方法について、統一したプログラムが提示されておらず、また、運動器の機能向上というと転倒予防・骨折に特化したメニューに偏りがちである。しかし、本研究の結果より、予防給付の対象者は全身の運動機能の低下による様々な不具合を複合的に抱えていることが想定されるため、対象者の運動器の機能を総合的に評価し、尿失禁がある高齢者には骨盤底筋体操を取り入れるなど、個別性の高いプログラムメニューを提供していく必要があると考える。
