项目摘要

1.性同一性障害の疾病概念が普及し医療制度が整備されたことによる変化を知るため、当事者と医療従事者への聞き取り調査、自助グループ集会や医療講演会でのフィールドワーク、医学論文やミニコミ誌等の文献調査を行った。まず、90年代の公的医療黎明期と比較して、当事者像が大きく変わったことがわかった。かつては希望の性別で社会生活を送る要望を持って受診する者が多かったが、現在は若年層を中心に、自分が何者でどうするべきか診断してほしいという、受動的な受診者が増えた。また、性別越境の医療化に疑問を投げかけるトランスジェンダーやクィアの活動も活発になっている。同時に医療側でも、典型的な性同一性障害だけでなく、ジェンダーに混乱する様々なケースへの、医学的理解や支援の重要性を指摘する声を聞く。性同一性障害医療や社会制度の確立だけでなく、現在は多様なジェンダーの問題へと課題が広がりつつあるといえる。2.性同一性障害医療は各科をまたぐ包括的チーム医療が必要とされるが、総合病院以外で提携や情報交換がどのようになされているのかは知られていない。そこで、形成外科ナグモクリニックの協力を得て、性同一性障害のホルモン療法を行っている医療機関の情報を収集し、アンケート調査を行った。回答から、医療機関が地方に非常に少ないこと、診断と治療のガイドラインへの認識が不十分な医療機関も存在することなどが明らかになった。また、地方の開業医への聞き取り調査の結果、性同一性障害に対する認識には医師によってばらつきがあること、当事者や知り合いの医師・教師の人づてで診療を開始するケースが多いことなどがわかった。3.フィードバックとして、性と人権ネットワークESTOと性同一性障害患者支援センターにおいて、タイにおける性同一性障害医療事情や性的マイノリティの自助・社会活動について報告を行い、医療機関リストの作成に協力した。
