世界18都市の外務省在外公館の邦人保護担当領事および医務官を対象として、海外渡航中に在外公館で保護された邦人精神科救急事例に関する半構造化面接を施行し、国境を跨ぐことによる精神医療サービスの分断の実態を明らかにした。対象事例のうち、半数が日本で治療歴があり、日本の医療機関からの紹介状を持っていた事例は皆無であった。対象の半数が現地で入院治療となり、帰国し治療継続となったのは入院者が8.8%、通院者が13.7%にすぎず、26.4%の転帰は不明であった。複数の治療分断の要因を検討した結果、国内外の治療者間の医療情報共有および帰国事例の空港での受け入れ体制整備が特に重要な課題であると考えられた。

The Ministry of Foreign Affairs of the 18 cities of the world is responsible for the protection of the consular and medical officers in the foreign residence, and for the protection of the psychiatric emergency cases in the foreign residence. The semi-structured interface is implemented and the state of the psychiatric service is clearly established. For example, half of the cases in Japan are treated, and the medical institutions in Japan are not treated. Half of the patients were admitted to hospital for treatment, 8.8% were admitted to hospital for treatment, 13.7% were admitted to hospital, and 26.4% were unknown. The results of the analysis of the main causes of multiple treatment breaks, the sharing of medical information between domestic and foreign practitioners, and the establishment of a reception system for international cases are particularly important issues.
