背景:がん患者は身体的にも心理社会的にも多くの負担を有しているが,それらに対する適切なケアが提供されていないのが現状である。目的:主治医が,患者の支持的ケアに関するニーズをどの程度把握しているかを明らかにする。方法:1.外来乳がん患者を無作為に抽出し,研究参加へのインフォームドコンセントを得た。2.患者に支持的ケアのニーズを評価するSupportive Care Needs Questionnaire,身体症状の程度を評価するEORTC-QLQ C-30,抑うつと不安を評価するHospital Anxiety and Depression Scaleの記載を依頼した。3.患者を診察した医師に対して,医師自身がその患者についてどのような症状やケアのニーズがあると感じたかを問う質問票の記載を依頼した。4.患者・主治医双方から得たデータを照らし合わせ,統計解析を行った。結果:最終的に408名の患者より有効なデータを得た。患者背景は女性100%,平均年齢56歳,進行がん(III, IV,再発)23%であった。またがん診断から調査時点までの期間は平均995日(中央値698目)であった。主治医による患者の症状の認識には,1.感度が低く,特異度が高く,陽性的中率も高い:多くの身体症状,2.感度が低く,特異度が中等度,陽性的中率が低い:不安,3.感度が高く,特異度が低く,陰性的中率が高い;抑うつ,という3つのパターンがあった。考察:主治医にとって,支持的ケアに関するニーズを把握することが困難であることが示された。それらの評価を援助するような介入が,患者のQOLを改善するために有用かもしれない。
