【背景】一般市民において、精神疾患やその治療に関する知識不足や偏見が、精神的負担に関する受療行動を阻害する重要な因子であることが示唆されている。【目的】がん患者における精神的負担の認識とその対処方法への意向を明らかにする。【対象】外来通院中の肺がん患者(CP)100名及び一般市民(LP)300名。【方法】対象を無作為抽出し、インフォームドコンセントを行った。CP群ではがん患者でかつうつ病を有している仮想症例を、LP群ではうつ病のみを有している仮想症例を提示し、仮想症例の状態、援助となる人・薬物・非薬物的対応について構造化面接を行った。またCP群では、精神的ケアへの抵抗感に関する質問票を施行した。【結果】仮想症例の状態をうつ病と正答した割合は、CP群において有意に少なかった(CP:11%,LP:25%)。仮想症例への援助として、CP、LP両群において、運動(CP:85%,LP:66%)、外出(CP:80%,LP:64%)といったうつ病治療としては非標準的な対処方法を有効と考える人が多かった一方、精神科病棟への入院(CP:16%,LP:16%)、抗うつ薬(CP:39%,GL:36%)といった標準的治療を挙げた人は少なかった。またCP群は、精神的ケアの必要性の判断が難しいこと、多忙な主治医への遠慮、薬物の副作用、などの様々な抵抗感を有していた。【考察】がん患者におけるうつ病に関する認識は一般市民よりも低く、一般市民同様に標準的治療よりも非標準的対処方法が有用だと考える傾向にあった。また精神的負担に対するケアについて、様々な抵抗感を有していることが示唆された。円滑な精神的ケアの提供に当たっては、十分な情報の提供などによってこれらの問題を解消する必要がある。現在本研究の結果を踏まえて、これらの問題を軽減するような患者教育用パンフレットを作成中である。
