α-シヌクレイオパチーである、パーキンソン病(PD)と、多系統萎縮症(MSA)でのREM睡眠行動障害とその発現背景となるREM without atonia(RWA)の頻度と経過・臨床的意義の比較ならびに、心筋交感神経指標(MIBG)所見のRBDを有するPDでの特徴について検討を加えた。その結果、MSAでは、運動症状発現前後にRBD症状が生じるものの、経過中に自然消失する者が多く、追跡調査においてRWAは増えるものの、RBDを有する症例の割合は増えないことがわかった。一方PDでは、運動症状発現後に徐々にRBD症例が増えていくこと、幻覚症状との因果関係が高いこと、追跡調査期間(2年間)では、RBD,RWAの動向に変化は無かった。MIBGの検討において、RBD症状を有するPD症例でのMIBG集積率は、認知症状を有する症例でのそれと同水準まで低下していた。以上より、両疾患は神経変性の過程の違いにより、RBD臨床症状が異なること、特にPDでのRBDは幻覚・認知症状と因果関係を有するものと判断された。

The background of REM sleep activity disorder and its occurrence in alpha-amyloid disorder (PD) and multiple system atrophy (MSA), the frequency and clinical significance of REM without atonia(RWA), and the characteristics of PD as seen in cardiac sympathetic index (MIBG) are discussed. Results, MSA, RBD symptoms before and after the onset of motor symptoms, RBD symptoms. On the one hand, the incidence of RBD increased after the onset of PD and motor symptoms, the causal relationship between phantom symptoms and RBD, and the trend of RBD and RWA increased during the follow-up investigation period (2 years). MIBG accumulation rate in patients with MIBG symptoms and RBD symptoms is low. The above mentioned problems are related to the process of mental changes, the clinical symptoms of RBD are different, and the cognitive symptoms of RBD are related to the causal relationship
