1)大腸の、 sessile serrated adenoma:SSA、sessile serrated polyp:SSP (SSA/SSP)は、(1)腺管底部の上皮の鋸歯状変化、(2)表層上皮の絨毛状変化、(3)腺管分岐の増加、(4)腺管の水平方向への変形、(5)拡張腺管、(6)腺管上皮の核偽重層 / abnormal proliferationの6所見によっての病理組織診断がなされうると考えられた。2)BRAF変異、フラグメント解析の結果からSSA/SSPはMSI陽性大腸癌と近い遺伝子背景を有することが示唆され、SSAがMSI陽性大腸癌の前癌病変である可能性が考えられた。SSA/SSPのBRAF遺伝子変異は、正常粘膜や管状腺腫より明らかに高率であったが、右側結腸のMSI陽性大腸癌よりは低率であった。従って、SSA/SSPでBRAF mutation陽性の症例に特に癌化のポテンシャルが高い可能性が考えられた。3)今後さらにメチル化異常について検討するため、メチル化アレイを行う予定である。メチル化アレイには、フラグメント解析で使用した、制限酵素処理済検体を用いる。マイクロアレイはより詳細なメチル化異常を検索することが可能であり、個別の遺伝子領域、非遺伝子領域でのメチル化、脱メチル化の程度が明らかとなる。それによって、これまでに報告されていない、新しいメチル化異常が明らかとなることが期待される

SSA/SSP's BRAF syndrome has a high rate of tubular adenoma in normal mucosa, and the right colon has a low rate of MSI-positive colorectal cancer. Cases of BRAF mutation-positive symptoms such as SSA/SSP are highly likely to become cancerous. 3) From now on, it will be determined whether the abnormality will be changed in the future.
