项目摘要

加齢黄斑変性は、高齢者の黄斑部に脈絡膜新生血管を生じる疾患で、本邦でも中途失明原因の上位を占めている。加齢黄斑変性に紫外線の影響があるかを調べるため、生涯に紫外線に曝露される量を顔面皮膚の皺量として定量化し、関連を評価したところ、少なくとも鹿児島県では紫外線曝露と加齢黄斑変性は関連があるかもしれないというデータが得られた。また、加齢黄斑変性のひとつのタイプであるポリープ状脈絡膜血管症の発症には脈絡膜の血管構築が関与している知見が得られた。さらに、この疾患の中には自然経過が良好な症例もあり、初診時の眼底所見と最初の2年間の視力経過が予後を予測する因子である可能性を見出した。また、網膜疾患と液性因子については、High Mobility Group Box Protein 1が、眼内の創傷治癒を誘導する網膜色素上皮の遊走に働く重要な伝達物質である可能性を見出した。
