项目摘要

【研究目的】本研究は、在宅で医療行為を必要とする患者が、安心して満足できる療養生活を継続できるための新医療専門職(米国と近似したナースプラクティショナー)の制度創設の提案に向けて、その根拠を示し検討することを目的とするものである。研究1年目は、在宅で医療行為を必要とする患者と、訪問看護指示書を基に在宅で医療行為を実施する看護師に面接調査を実施した。研究2年目の本年は、在宅で訪問看護指示書を書いて診療に関わっている医師を対象に、在宅療養患者の診療に関わる中で経験する医師の"困難さ"の特徴をしめし、在宅療養患者の診療の継続における問題点と課題を明らかにすることを目的とした。【研究方法】調査期間:H23年12月～H25年1月、調査対象:訪問看護師に訪問看護指示書を書いて在宅療養患者の診療に関わっている(関わったことのある)医師8名。データ収集方法:勤務している病院又は診療所において、同意の得られた医師に、30分から40分程度、在宅療養患者の診療に関わる中で経験する医師の"困難さ"をテーマに自由に語ってもらう非構成的面接を行った。承諾のもと面接内容をICレコーダに録音し、後日逐語録を作成し分析素材とする。分析方法:ベレルソンの手法を参考に、訪問看護指示書を書いて在宅療養患者の診療に関わる医師の"困難さ"に焦点をあてて内容分析を行い、"困難さ"の特徴を明らかにし課題を検討する。【倫理的配慮】研究者の所属する大学の倫理審査を経て実施した。【研究結果1調査対象者の背景:病院医師2名、診療所医師6名(内1名在宅療養支援診療所医師)。全員男性。医師としての経験年数は、20年(2名)から40年(1名)。訪問看護指示書を書き始めてからの期間は、10年以上の者が4名、4年以上10年未満が4名。現在訪問看護指示書を通して診療している患者数は、10名以上の患者に書いている者3名、4名から6名の患者に書いている者3名、現在患者0名が1名。現在、逐語録を基に、医師の"困難さ"について内容分析の継続中である。

[Objective] This study aims to establish a new medical system for patients who need medical care at home, and to provide a safe and healthy lifestyle for them. 1 year study on the need for in-home medical behavior, visit nursing instructions, in-home medical behavior, and in-home nursing practice. 2-year study on the characteristics of physicians '"difficulties" in the diagnosis and treatment of in-home nursing patients [Method] Survey period: December 2023 to January 2025.
