项目摘要

本研究の目的は、島嶼県沖縄と太平洋島嶼国で展開されている高度実践看護師教育との融合を目指した、グローカル教育を基盤とする島嶼看護の継続教育を構築することである。令和4年度は、太平洋島嶼国や国内で活躍するナースプラクティショナー（以下、NP）に対する継続教育の実態調査を行うにあたって、国内外でのNPに対する継続教育の文献検討を実施した。また、国外の島嶼国の一つとしてハワイで活躍するNP2名にヒアリングを行い、継続教育の概要や更新について情報収集を行った。文献検討では、約5年間（2018年～2023年4月）までにおける文献をNPと継続教育のキーワードで検索した。その結果、国内文献1件、国外文献127件であった。国外文献のうち64件がAdvence in Skin & Wound Careに掲載されたもので、高度な皮膚・損傷ケアに関する内容が多かった。64件の内訳は、創傷ケア23件、褥瘡ケア12件、潰瘍ケア6件、皮膚炎・損傷など6件、フットケア3件、悪性黒色腫2件、ストーマケア2件であった。その他、血管炎、リンパ浮腫、強皮症、帯状疱疹、瘢痕・ケロイド等各1件であった。37件は教育プログラムやトレーニング内容を作成し、その効果を評価した研究、15件はあるテーマに関する知識や態度・行動を調査し、そのテーマの継続教育の必要性について報告された研究であった。11件は、最新の情報についてまとめ、知識提供を通して情報発信を行っていた。8件が小児NPに特化した文献で虐待、いじめ、環境、疾患に関する論文であった。国内の文献は1件のみで、修了後のNP到達目標と評価表作成に関する研究であった。ハワイでは、NP免許更新が2年間ごと、さらにNPの認証更新が5年ごとで、150時間の研修、その内薬理学70時間が含まれていた。国内のNPのニーズを把握し継続教育の内容を精査していくことが必要である。
