東京大学医学部の協力の下、「診療報酬制度が医師の診療行為上の意思決定に与える影響の構造推定」に必要な診療行為及び保険請求状況の個票データの加工を行うとともに、推定に必要なモデルの構築とその理論的性質の分析を行った。この個票データは、(1)出来高報酬制度の下で医師が患者に不必要な診療行為を行っていたか否か、(2)行っていたとすれば、どの程度の規模で行われていたか、(3)その結果として患者の厚生がどの程度損なわれていたか、(4)DPCに基づく診療報酬制度によってこの問題がどの程度解決されるか、といった問題を統計的に分析するために必要なものである。この分析に当たり、離散選択のrandom-coefficient modelを動学的なモデルに拡張する作業を行った。このようなモデルとしては、これまでには異なる時点の意思決定を独立なものとして扱うようなモデルしかなかったが、今回はある時点での意思決定がその時点以後の意思決定影響を与えている可能性を許している点が特徴である。このモデルを用いることにより、同一入院患者に対する入院期間中の医師の診療行為や退院の意思決定がどのような要因によって決定されていたのかをデータから構造推定することが可能になる。特に、各診療行為への報酬の変化と、入院基本料の変化が、医師の意思決定のどのような影響を与えていたかを区別して推定することが可能になる。今後の課題は、このモデルを推定するための効率的な方法を構築することである。
