小児がんは,治療法の進歩により,治療を受けた75～80%の患者において完全治癒が期待できるようになった。しかし,小児がんは,身体的・精神的な成長途上に発病するため,疾患および治療が患者にもたらす身体的,心理社会的な悪影響は,成人がんよりも大きいことが指摘されている。本研究では,小児がん患者の日常生活における不安や困難を明らかにし,退院後の小児がん患者の心理社会的状態の特徴を検討することを目的とした。本年度は,入院治療を終えて日常生活を送っている小児がん患者を対象に,退院後の困難感に関するアンケート調査を実施した。回答の得られた小児がん患者32名のデータから因子分析を行い,退院後の困難感の因子構造の検討を行った結果,(1)日常生活における活動の困難さ,(2)病気を経験したことで生じた周囲との関係性への戸惑い,(3)病気により変化した自身の身体の様子の3因子が抽出された。これらの困難感に関して,女性の方が対人関係への困難感が強いこと,思春期に発症した患者や現在の年齢が高い患者の方が,日常生活における活動に困難感を抱いていることが示された。また,退院後の困難感が強いほど日常生活への辛さを感じていることが示された一方で,退院後の困難感と日常生活への満足感には関連がみられなかったことから,退院後の困難感を低減させるだけでは患者の満足感は向上しないことが示唆された。以上のことから,患者の属性を考慮した長期にわたるケアが必要であること,また,退院後の生活をより充実させるためには,困難感の低減と満足感の向上の両面から検討していく必要があることが示唆された。
