项目摘要

平成28, 29年度に日本全国から13の集中治療室の協力を得て、多施設共同AKIレジストリ(JAKID)を構築した。このレジストリにはのべ2421件、2292人のICU入室中のデータ、退院時の転帰データが登録された。平成30年度はこのデータベースを用いて、重症病態の評価と有効な治療法の探索に関する研究を行い、学術集会、国際学術誌への報告を行った。具体的には、AKI患者へのカルペリチド投与の有効性の検証、重症患者の臓器障害スコア(SOFAスコア)へのAKI国際診断基準の適用の有用性、について、国際学術誌に発表した。また、SOFAスコアへのAKI国際診断基準の適用の有用性、心臓手術術後患者の尿量がAKI診断に与える影響、についてはそれぞれ、欧州集中治療医学会、米国集中治療医学会の年次学術集会で結果を発表した。さらに、重症患者の血行動態管理によるAKI発症の予防、重症患者に対する造影剤投与の影響、に関する検証研究は、解析を終了しており、今後の国際学会での発表・論文化の準備を進めている。また、重症患者診療は常に治療制限や積極的治療の手控えなどの倫理的問題を伴う。ICUでは急性期特有の診療スピード、短時間の患者家族との関わり、など、携わる医療者が抱える心理的負担も大きい。急性腎傷害患者に対する血液浄化療法も体外循環を要し、患者への心身の負担は大きく、治療制限が考慮される代表的医療介入のひとつである。本研究では平成29年度に集中治療従事者に対して実施した倫理的苦悩に関する調査研究において得られた調査データを分析・解析し、日本集中治療医学会において発表した。現在論文投稿の準備が進んでいる。

In the acute stage of ICU, the patients' families have a long-term illness, and the patients in the family have a long-term care, and the patients who carry the medical care provider have a lot of psychological stress. The method of acute injury to the patient, the transformation of the blood, the importance in vitro, the mind and body of the patient, and the treatment of the medical intervention represented by the medical intervention. The purpose of this study is to focus on the treatment of people in Pingcheng in the 29th year, and to study the pain and suffering of the patients in Pingcheng in the 29th year. At present, we are ready to submit articles and prepare for the final review.
