项目摘要

言語優位半球側に病変の主座をもつ5症例の脳腫瘍患者に対して、機能的MRI(以下fMRI)と覚醒下術中機能マッピングを併用した腫瘍摘出術を実施した。病変部位は3症例が前頭葉にあった他、頭頂葉と側頭葉が1症例づつであった。頭頂葉病変症例では摘出が白質に及んだところで音節性錯語が出現したことから、深部の腫瘍摘出は断念せざるを得なかった。また側頭葉病変症例においては、覚醒直前に出血源不明のクモ膜下出血をきたしたため腫瘍摘出術を中止した。前頭葉病変をもつ3症例では予定通りの腫瘍摘出術を実施できたものの、1症例では術中の覚醒が不十分であったことから直接皮質電気刺激による言語評価が困難であり術後に超皮質性運動失語をきたした。また他の1症例においては病変に補足運動野を含んでいたが、過去の知見より摘出に伴い失語を呈したとしても一時的であることが予想されたため摘出に踏み切ったところ術後に補足運動野失語をきたした。しかし2症例共に症状は一過性で間もなく概ね術前の言語機能の水準まで回復した。以上の経緯より、前頭葉病変を有していた3症例について術後2ヶ月目にfMRIの再検を行った。内2症例では手術前後で著変を認めなかった一方、1症例では聴覚入力動詞産生課題において下前頭回の活動の左右差が不明瞭化した。この2年間で脳腫瘍9症例に対して術前後にfMRIによる評価を行うことが実施でき、3症例において術後に若干の賦活領域の変化を認めた。しかし、この3症例はいずれも研究期間内に再発し再手術に至ることはなかったため、この賦活領域の変化が手術に関連した言語関連領域の移動を反映しているのか否かを判断することはできなかった。

The deep swelling is picked up by the broken line A case of lateral lobe disease was identified and the source of bleeding was unknown. 3 cases of frontal lobe disease were performed with tumor extraction, 1 case was performed with intraoperative wakefulness, 1 case was performed with direct cortical electrical stimulation, and 1 case was performed with postoperative hypercortical motor aphasia. The first case is to supplement the movement field with the knowledge of the past, and the second case is to supplement the movement field with the knowledge of the past The symptoms of the two cases were transient and the level of speech function before surgery was restored. 3 cases of the above diseases were examined 2 months after operation. In 2 cases, there was a change before and after surgery, and in 1 case, the difference between the left and right sides of the activities of the front and back of the lower part was unclear due to the problem of verb generation.
