项目摘要

小児の平等な権利を保証する観点から、2016年4月に小児がん陽子線治療が保険収載されたことは促進要因となり、施設間連携の重要性が高まっている。当院の小児がん集学的治療において陽子線を適時に利用することを目的とした診療連携の現状を分析した。【方法】対象は2019年4月から2021年5月の間に当院で治療を開始後に陽子線施設と連携した連続10症例である。陽子線治療施設への連絡方法等、治療日程遅延の有無、有害事象等について分析した。【結果】対象10例は、髄芽腫4、再発上衣腫2、Ewing肉腫2、pineal parenchymal tumor of intermediate differentiation 1、Hodgkinリンパ腫1であった。小児がん拠点病院・連携病院体制に基づく関東甲信越地域内の連携は5例で、他の5例は地域外施設との広域連携であった。すべての症例で日程の遅延なく安全に陽子線治療を利用できた。【結論】広域的視野により陽子線施設との連携を促進し、患児・家族を支援することで、陽子線を利用した小児がん集学的治療の機会均てん化が実現する可能性がある。
