エンド・オブ・ライフ期の非がん疾患患者は、生命予後の予測が困難であることから、病状の急変に伴った早急な意思決定を求められる。特に高齢の非がん疾患患者に対する緩和ケアの体制は未だ十分とは言えず、早い段階から治療に対する意志決定への備えが重要となる。本研究は、エンド・オブ・ライフ期にある高齢の非がん疾患患者に対する意思決定のニーズと課題を明らかにするために文献検討とインタビューを実施し、質問紙調査により意思決定の実態を把握したうえで、以前より分担研究者らとともに開発しているアプリを活用して、在宅において患者の意思決定を支援システムの構築と検証をする計画である。研究の実施は、高齢の非がん疾患患者の意志決定に求められるニーズを明らか（フェーズ1）にし、そのために求められる支援体制を検討する（フェーズ2）とともに、適切な支援を行っていくためのシステムの構築と検証を行う（フェーズ3）の3つのフェーズに分けて計画した。本年度は、フェーズ1に該当している。高齢の非がん疾患患者の治療の選択に伴った意志決定に求められるニーズを明らかにすることを目的として、脳血管障害患者の意思決定について文献検討を行った。治療にかかわる意思決定支援が行われていることが分かったが、その一方で、意思決定支援として取り上げられている場面は限定的であり、また、具体的な意思決定の方法や支援の具体的な内容について記載されている文献はほとんど見当たらなかった。今後さらに、対象疾患を広げていき、意思決定の現状と課題を明らかにしていく予定である。
