本研究の目的は、①鹿児島県において、精神科入院患者が生まれ育った離島へ帰りその人らしく生活するための退院支援を明らかにする。また、②都市部、離島、へき地の3地域において、精神障害者の生活へ影響を与える要因を調査し、地域特性による影響の有無について明らかにすることで地域特性を踏まえた精神科退院支援の方策を確立することである。①についての研究実施計画は、倫理審査の承認を得て、2022年度から2023年度に面接調査を実施し、2024年度に成果を発表する予定である。2022年度の①の成果は、研究の倫理審査の承認を得たのち、2か所の調査協力施設に調査依頼し、離島に退院する患者の退院支援について面接調査を実施している段階である。離島への退院者への特有な支援の方法が必要であることを見出しつつある。②についての研究実施計画は、倫理審査の承認を得て、まずは離島の現状調査を行い、その後2023年度にかけて都市部との地域差についてアンケート調査を行っていく予定である。②の2022年度の成果として、離島で生活する精神障害者のQOLに影響する要因を明らかにした。また、精神科病院のない離島の精神障害者の生活を支援する多職種の困難について調査し学会発表を行った。その調査の結果、離島での支援では、病状悪化時の対応や地域生活の拡大に向けた支援の困難があり、保健・医療・福祉の連携体制つくりや社会参加を促す環境整備の必要性が示唆された。これらの2022年度の成果より、離島への精神障害者の退院支援には、サービス資源が限られた遠隔地特有の退院支援の方策や、離島でその人らしく生活できるための地域力を活かした支援について、今後の調査でより具体的な内容を明らかにすることが重要であると考える。

The purpose of this study is to provide support for the rehabilitation of psychiatric patients admitted to Kagoshima Prefecture. 3. Investigation of the main factors affecting the lives of mentally disabled persons, and establishment of policies for psychiatric hospital support based on regional characteristics. (1) Research implementation plan, approval of ethics review, implementation of face-to-face investigation from 2022 to 2023, and presentation of results in 2024 are scheduled. In 2022, the results of (1) the recognition of the ethical review of the study,(2) the investigation cooperation implementation,(3) the investigation compliance, and (4) the discharge support for patients discharged from the hospital in the outlying islands. A unique support method for those who leave the hospital on the island is necessary. (2) Research implementation plan, approval of ethics review, implementation of status survey on islands, and implementation of regional difference survey by the Ministry of Urban Affairs in 2023. (2) The main causes of QOL impact on mental health disorders in 2022 are as follows: The Institute of Mental Health and Social Welfare has been conducting research on various kinds of difficulties in supporting the lives of people with mental disorders in psychiatric hospitals and outlying islands. The results of the investigation, support for the islands, difficulties in supporting the development of regional life, the integration of health care, medical care and well-being, and the need for social participation in promoting environmental protection are also discussed.
