项目摘要

ICTを活用した在宅医療支援モデルの地理的条件を解明するための事例の選定と調査を実施した。都市部では，潜在的な連携先である診療所は多数存在するが，従来競合関係にあったことから，信頼関係の構築が連携の条件となる。そこで，医師会や在宅療養支援診療所が，信頼に基づく草の根的な診療ネットワークを形成することが多い。そこで，京都市における在宅医療チームを先進事例と位置付け，潜在的な在宅医療ニーズを検討するとともに，多職種連携による在宅医療体制の空間特性を検討することとした。コロナ禍ではあったものの，関係者へのヒアリングを実施することができた。その概要は以下のとおりである。京都市西京区に立地する在宅療養支援診療所では，訪問看護師や介護福祉士，薬剤師，医療事務といった医師以外の職種が，医師とペアを組んで患者宅を訪問し，業務を分担している。京都府医師会が推奨するSNSの活用が，チーム内の多職種による問題の把握と対応を可能にしている。一方，コロナ禍では専門チームが別途組織され，地域基幹病院や入院医療コントロールセンターと連携することで，自宅療養中の高齢者の症状に応じた訪問診療体制を構築した。新たな組織内外の一時的な関係性は，市場原理とは異なる価値体系に基づいており，パンデミックによる突発的な需要にも有効であった。他方，平時と非常時とでは，形成される在宅医療体制のローカル・ガバナンスと，そこに関与するアクターの行動原理が異なることが示唆された。以上の成果は，「京都市における在宅医療体制の地域的展開」として学術雑誌にて公表した。
