高齢者の増加に伴い、心不全、とくに左室駆出率の保たれた心不全（HFpEF）が増加しているが、病態の多様性のため、有効な治療法は未だ確立されていない。近年、HFpEF患者における左房機能と予後の関係が報告されているが、その評価方法は確立されておらず、実臨床での応用には至っていない。HFpEFにおいて左房機能を評価し、左室拡張機能に加えて左房機能に着目しHFpEF患者を層別化すること、さらに心エコー図スペックルトラッキン法による左房機能評価に関する現在の問題点を洗い出し、左房機能評価の確立・普及を目指すことを目的とする。さらに、心臓MRIにより、左房機能、左房壁の組織正常評価の可能性についても検討する。2022年は、後向きおよび前向き登録研究の研究計画書を作成し、倫理委員会で承認を得た。その後、後向き登録に関して、今までに運動負荷心エコー図検査を施行した患者から、選択基準を満たす患者を抽出した。今後これらの症例とコントロール症例の解析を進める予定である。また前向き登録に関しては、2023年4月より登録を開始した。さらに、今後の研究の基礎データとして、当院のHFpEF患者の、運動負荷心エコー検査やナトリウム利尿ペプチド系バイオマーカーのデータなどをまとめ、学会などで発表した。心臓カテーテル検査の右心カテーテル圧データの肺楔入圧のV波から、Stiff LA症候群と診断された症例19例とコントロール症例55例における、運動負荷心エコー図検査に関するデータをまとめ、左房ストレインを評価することでStiff LA診断や心房細動発症リスクを層別化することができ、その後の心血管イベントのリスク評価に有用であることを示した。左房機能の観点からHFpEF患者の心不全治療薬に対する反応性や予後を見直すことにより、HFpEFの個別化治療へとつながり、患者の予後改善・医療経済の負担軽減が期待できる。
